For more than two decades, CCM has served vulnerable populations throughout Los Angeles County by developing innovative community-based solutions that integrate housing, health, employment, education, recovery support, and community connection.
The organization works collaboratively with government agencies, healthcare providers, faith-based institutions, community organizations, and private-sector partners to expand access to opportunities that foster self-sufficiency and long-term success.
